The Medallion of Karabor is an epic necklace that provides stamina and shadow resistance. It was originally held by draenei priests in order to allow them entrance to the Temple of Karabor. Now it serves the same purpose for adventurers seeking to challenge Illidan Stormrage, who has taken residence in the Temple of Karabor - now called the Black Temple.

The Medallion of Karabor is mentioned as a quest-item from Akama at the Warden's Cage, west of the Black Temple. In a long quest line beginning with the Aldor or Scryers (depending on the player's faction), the player is sent to the Ata'mal Terrace, located on the Temple's northern battlements, to obtain the Heart of Fury, one of the shards of the Ata'mal crystal. Akama places the Heart of Fury into the empty socket of his Medallion of Karabor and instructs the player to bring it to A'dal (Akama's Promise). Afterwards, a Broken named Seer Olum is opened up in Serpentshrine Cavern, where he reveals that the Ashtongue's secret - their intent to overthrow Illidan - has been uncovered by Lady Vashj's naga. Then under orders from Illidan himself, Akama's Ashtongue are sent to Tempest Keep to slay Al'ar, Kael'thas Sunstrider's phoenix. Lastly, the players travel to the very gates of the Temple itself and do battle with Illidari demons, allowing Akama and Maiev Shadowsong to enter the Temple to later aid you in the battle against Illidan. The naaru Xi'ri will then give you the Medallion.

The Medallion of Karabor presumably works like a similar necklace-attunement, the Drakefire Amulet (Onyxia's Lair), in that you must have it in your inventory but not necessarily be wearing it, in order to enter the Black Temple.

After completing the quest The Fall of the Betrayer, the Medallion becomes the Blessed Medallion of Karabor which, prior to patch 2.4, allowed entry into the Black Temple. It also allows the player to teleport directly to the doorstep of Illidan's fortress. The medallion is no longer needed to enter the Black Temple.
